I understand the confusion. There's no 4/4 on the image you provided, because the image is from the ending of the song, and the 4/4 (time signature) would have been at the beginning of the music. I included the 4/4 because I didn't clip the excerpt from the full song. I made the fragment myself and included the time signature for clarity. For your purposes it can be done with or without the 4/4--your choice. Someone who reads music would understand it either way.
The note at the very end has nothing to do with the melody for the words, so I don't recommend including it.

It's close.
1. You need the triplet markings over the quarter notes--the horizontal, sort of brackets, with the 3 in the middle. There are two of them.
2. For a tattoo, you could probably drop that last note that's "tied" to the one before it. It's more accurate the way you have it, but you could get away with the way I did it.
3. You didn't include the bar lines (the vertical lines between some of the notes). Again, for the purposes of a tattoo, you could probably get away without them, but it's more accurate with them.
4. You have a brace ({) at the beginning of your staff. Braces are for linking two staves together, so you don't typically see a brace on a single staff like that.
If you want, I can make you one more like yours, with no bar lines and no 4/4 time signature and with the extra note at the end. ?? Even the brace, if you really want it.
